Confidentiality Clause
All of the information provided by MediVet-America. to the Client during the term of this Agreement in the form of Intellectual Concepts, Intellectual Property, Marketing Methodologies and Marketing Strategies and any other promotional or marketing literature remains information proprietary of  MediVet-America. and shall not be used other than as expressly provided in this Agreement without the written authorization of  MediVet-America.

Indemnity Clause
MediVet-America. is the supplier of the Adipose Stem Cell Procedure Kit to Veterinarians. MediVet-America provides a protocol with the Adipose Kit that is to be adhered to. I understand that the liability falls with the Veterinarian Clinic. Any legal or other liability that occurs as a result of or during the procedure is solely the responsibility of the Veterinarian Clinic and not  MediVet-America.

Third Party Processing
MediVet-America has approved and appointed Official MediVet-America Processing Centers for the convenience of veterinarians who do not possess and who prefer not to purchase the in-house processing equipment. By signing this form I affirm that I will not seek or accept requests for processing from third party clinics or practices, but will refer all such requests to the Official MediVet-America  Processing Centers.

IP Surcharge: MediVet incurs intellectual property charges for the use of Adipose Stem Cell Procedure Kits.
